Thanks to generous funding from the Centers for Medicaid and Medicare Innovation, the Southwest Health Collaborative has been able to develop tools for care coordinators in Regions 3 and Regions 4. We understand the crucial role that care coordinators play in today’s complex healthcare environment. Through our work, we aim to support enhanced communication between key care coordinators by promoting connection, shared understanding, and opportunities to grow. The Treasure Valley Care Coordinator Directory IDCarenetwork.org is a tool for care coordinators to develop a professional network and connect to communicate along the same paths that patients move in the course of their care. We also offer learning collaborative sessions for local professionals. The schedule as well as previous presentations are posted to our website.
