As part of the SHIP, three cohorts of primary care practices will receive support in Patient Centered Medical Home (PCMH) transformation. Practices receive technical assistance in transformation from a local Quality Improvement Specialist based out of the public health district and a Practice Coach from Health Management Associates. Additionally, practices receive assistance with connecting to Idaho’s health information exchange, the Idaho Health Data Exchange (IHDE).
